CONSENT & RELEASE AGREEMENT

I accept your invitation to participate in the program(s) specified and hereby authorize you and your affiliated stations and cable systems to use my name, biography, likeness, voice and performance in the production of the program(s) and for the purpose of publicizing the program(s). I further authorize you to edit these programs and you and your affiliated stations and cable systems to make recordings of the program(s) and to use and re-use the program(s) and recordings of it, in whole or in part, for radio and television broadcasting, cable casting, audiovisual and closed circuit exhibition, internet broadcasts, rebroadcasts, program offerings and sales and other electronic, mechanical, or printed distribution throughout the world. I also understand you have no obligation to use the program(s). 

I agree, if by reason of my statements and actions on the program(s) or materials furnished by me for the program(s) or materials furnished by me for the program(s), there is any claim or litigation involving any charge by third persons of violation or infringement of their rights, to hold you and your affiliated stations and cable systems harmless from liability, loss or expense arising from such claim or litigation. 

I further agree that my appearance or participation in the program(s) confers on me no ownership rights whatsoever, and I consent to PTCB’s marketing of the program(s) in all its markets and venues for compensation without paying any compensation to me. I agree that I am to receive no compensation for my appearance on or participation in the program(s) when originally broadcast or during any later broadcast or other use. 

I further agree to hold Prime Time harmless of any damage to or loss of materials I furnish for the program(s) and submit the materials at my own risk.
